LINKCARE aims at offering a viable, innovative, integrated health care service for chronic patients, and providing appropriate support for information to health professionals in their activity (monitoring and management) and to pave the way for the implementation of new care models. LINKCARE, as a service provider aims to contribute to the change in the way healthcare is delivered to chronic patients (i. e. cardio, psychiatric, etc.) to overcome the problems associated to fragmentation of care. The project will focus, not only on the technical suitability of the generic platform, but also on the various key aspects involving reengineering of the healthcare system. The business case will encompass a services’ portfolio pointing towards an adoption of novel integrated care services by healthcare systems at European level.
